Steps for the Theft Claim
- Intimate the theft claim from http://mobisecurity.net/claims/ and get Claim intimation acknowledgement reference number.
- Download the following forms from http://mobisecurity.net/claims/
- Theft Claims Declaration format
- Police Complaint form
- Application for barring SIM card form
- Submit the Police Complaint form duly filled to the nearest police station and get their numbered acknowledgement (CSR / Daily Diary Report)
- Fill the form for barring SIM card of the lost/stolen mobile and submit it to the nearest gallery of service provider for its deactivation. Get the acknowledgement on the same form.
- Finally, Prepare all the following documents :
- Theft Claim Declaration duly filled
- Daily Diary Report acknowledgement number (also known as CSR) from Police Station.
- Acknowledgement for barring SIM card duly signed and stamped from mobile service provider (for e.g. Airtel, Vodafone etc.)
- Original mobile phone purchase invoice.
- Original certificate of insurance received from the Insurance Company.
- Claim intimation acknowledgement reference number generated through above link.
- Download NEFT letter for settlement of claim and fill up details.
- Cancelled cheque of insured person’s bank account.
- Letter of Indemnity
- Post all the above documents to the following address

Mumbai-400021 - Claim amount will be deposited by insurance company through NEFT in 15 working days after the receipt of all documents at given Edelweiss Address.
Steps for Accidental /Liquid Damage claims
- Intimate the Accidental /Liquid Damage claim from http://mobisecurity.net/claims/ and get Claim intimation acknowledgement reference number.
- Download the Claim form and Repair estimate letter from http://mobisecurity.net/claims/
- Get Estimation letter from authorized service center and send an email to [email protected] with claim intimation reference number in subject and attach the following forms.
- Mobile phone purchase invoice
- Photograph of damaged mobile handset with IMEI number – send it to email. Claim intimation reference number.
- Estimation letter from authorized service center.
- You will receive an email from edelweiss on your registered email about approval / rejection in 3 working days.
- Post approval, you can get your mobile repairs done, Send the following documents to Edelweiss at Mumbai address which mentioned at the end of this document.
- Original mobile phone purchase invoice
- Original certificate of insurance received from the Insurance Company
- Claim intimation acknowledgement reference number generated through above link Or Claim intimation letter/email if claim is not intimated through the portal.
- Estimation letter from authorized service center.
- Original Bills for repairs/replacement
- NEFT letter for settlement of claim with cancelled cheque of insured (letter format downloadable from portal)
- Claim amount will be deposited by insurance company through NEFT in 15 working days after the receipt of all documents at given Edelweiss Address.

Steps to claim Personal Accident Insurance
In the event of Accidental death the following documents are required to be submitted:
- Intimate the Personal Accident Insurance claim from http://mobisecurity.net/claims/ and get Claim intimation acknowledgement reference number.
- Collect the following documents and sent it to Edelweiss Address mentioned in this document.
- Post-mortem Report.
- F.I.R./Police Report.
- Punchnama.
- Departmental Inquiry, if any
- Report of Doctor/Hospital/Nursing Home
- Death Certificate - Confirmed
